Introduction

Logan Hospital in Queensland, Australia faces high rates of minimal trauma fracture (MTF): a burgeoning cohort of patients aged over 65 years, socio-economic disadvantage and rapid population growth (2). In individuals referred with minimal trauma fracture, we aimed to: (1) quantify documentation of clinical diagnosis of osteoporosis and (2) identify rates of secondary fracture prevention assessment and osteoporosis treatment initiation within twelve months (2).

Method

A retrospective audit was performed of adults aged 50 years and above who experienced MTF and reviewed through the Logan Hospital orthopaedic fracture clinic between July-December 2023.

Results

157 MTF patients were identified, of which 130 were analysed with a median age of 68 and 81% female predominance. Fractures sustained included 70% distal radius, 25% proximal humerus and 5% hip. 35% had a documented secondary risk factor of osteoporosis. 107/130 (82%) had no prior diagnosis of osteoporosis. Of 107 MTF individuals with no prior osteoporosis diagnosis, 17 (16%) were diagnosed with osteoporosis. All 90 patients without an osteoporosis diagnosis did not receive secondary assessment or initiate treatment within 12 months, while the majority of those with a diagnosis (94%) did initiate treatment within 12 months (p < 0.001).  

Conclusion

Major gaps in the management of osteoporosis following a minimal trauma fracture have been identified at our hospital including lacking recognition of osteoporosis diagnosis and subsequent lack of secondary assessment and treatment. This audit has provided the basis for starting a fracture liaison service which will be reported on in the future.
