The past year has seen an evolving landscape in the field of osteoporosis, and this presentation aims to highlight key articles, position papers and hot topics from the past year, focusing on practice changing clinical updates.

Osteoporosis continues to advance beyond a diagnosis based predominantly on bone mineral density towards more individualised approaches to assessment and management. Updates will include fracture risk assessment and risk calculators, novel imaging approaches, and the growing potential for AI-based opportunistic analysis of routine imaging to identify osteoporosis and fractures. Emerging perspectives and data on the role of race and ethnicity in fracture incidence, risk assessment and management will be highlighted, alongside the ongoing debate around goal-directed osteoporosis therapy and potential treatment targets.

New evidence is also informing how both established and newer osteoporosis therapies may be used, such as intermittent zoledronic acid treatment regimens, as well as anabolic therapy in secondary causes of osteoporosis. Practical challenges in osteoporosis management remain equally important in recent literature, with updates on denosumab discontinuation, drug holidays, MRONJ and emerging concerns regarding the concurrent use of intravenous iron and antiresorptive therapy. Recent position papers on specific cohorts, such as pregnancy and lactation, advanced chronic kidney disease, and breast cancer, will be discussed.

Beyond pharmacotherapy, the presentation will highlight recent literature addressing the challenges facing fracture liaison services, the persistent osteoporosis treatment gap, and opportunities to integrate digital health tools into models of care. Finally, recent evidence on exercise will be covered, alongside the evolving intersection between weight loss, GLP-1 receptor agonist therapy, and musculoskeletal health.
